Inquiry on diploma supplement

Dear Campus-Center Team,
I am Daren from University of Science and Technology of China, and I am in the last semester of the bachelor program. When applying for the international master program in physics, I noticed that a document called diploma supplement (or program description) is required. Due to the difference in education systems, I am not sure about which certification this document refers to. Can you inform me where I can get such kind of document? Or should I write it myself?
