Adding middle name in the offer letter

Hello,
I am an international applicant and have been selected for the masters programe in physics for the winter semester 2022-23.
While applying i did not mantion middle name(father's name) in the first name. But in my passport and all other documents my full name is 'MITULBHAI NANDLAL AKBARI'.
But in my offer letter it is 'MITULBHAI AKBARI'.
During application of visa i found out that all the documents should have same name mantioned in them.
Please help me, how can i make changes in my offer letter and enrollment application?
What is the procedure?
