Question related to filling of Matriculation Form

Hello Team,
My application ID is 1211355. I have received a letter of acceptance from the university on 9th May and I am supposed to send the documents such that it will be received by the university before 23rd May.
I have below doubts in filling the matriculation form.
1. Do I need to necessarily mention the name and address of health insurer in section 4?, Can I mention these details after sending the matriculation form as soon as I get them. because to include these details I might require some time and I fear of missing the deadline.
2. Also in section 7 of Matriculation form, do I have to provide the academic history of each semester of my Bachelors which I did from India, from University of Pune?

I have one more question.
In section 7, for column "Degree Program", Do I mention 'Bachelor of Engineering, computer' for all 8 semesters? OR do I mention 'First year(Bachelor of Engineering, Computer)' for first 2 semester and like wise for remaining( as second year, third year etc).

If there is a different way apart from the above two, please let me know.
All the eight semesters are in BE, Computer and there was no leave or break in between. In this case, can I select "On leave" Column as NO.?

Dear Mr Motiwala,

If you studied in the same program during the whole time you can put the same description for the program in that column for each semester. "Bachelor of Engineering, computer" would be fine.

thanks again for the reply. i have sent the documents already to the university. Please let me know if notary attested certificates are fine or do I need to get it attested from consulate of germany.
I am really afraid to miss the deadline.

Thank You for clarification. I have sent my documents and were recieved at 14.05.2018 at the university, as per my courier service but I havent got any confirmation from the university. Will this be updated on stine for recipt of documents or will I get a mail from the admissions team ?
