Confirmation of Doctoral Degree Conferral

Hello,

I am a doctoral graduate from Uni-Hamburg, (2002). I would like a form to be signed by the University to confirm that I graduated from the University. Could you direct me to the office (email address) where I can submit the completed form for a signature? The signed document has to be returned to an institution in the United States.
